We used to have unique title and descirption meta tags. All the meta tag information still can be viewed in admin control panel.
I do not know what happended but we now have all same meta tag as homepage. Please help me restore unique meta tags.

Well, if it "used to" show unique title/description, and now it doesn't ... then ... something has been changed.
Tossing the ball back in your court to perhaps enumerate all the things you've changed since it was last working. Perhaps you might also explain what version of Zen Cart you have, what version(s) it has been upgraded from, and when, and a list of addons/contributions you have installed, as well as your PHP version.

We are using:
Zen Cart 1.3.7
PHP Version 5.25 (Zend 2.2.0)
I think PHP version was upgraded by web host from 4. to the current 5.25
We purchased seo rewrite from web host to make the page to have all static pages, but they say it only changes URL and it does not affect anythign else.
Maybe I am missing core file that is needed to display unique meta tag?
What *exactly* did they do to make this happen?

I just wanted to post update on my problem. I could not figure out what was wrong so I went to ScriptLance and the guy fixed the problem for me. The problem was in the code [FONT=Arial]includes/templates/template_default/common/html_header.php[/FONT]

Well, yes, that's one of many places the problem could stem from.
And it's likely whoever attempted to do your SEO "enhancements" for you who changed that file.
