problem with collect_info.php

I'm adding a few more options to my product collection pages...
i need to add:

supplier (from drop down)
supplier product url (text box)
supplier identifier (textbox)

these options are displaying correctly, but when i go to save the item, i get an error..

please pm me if you would like username and password to my admin (its stil in text mode!)

i've attached the php file incase theres anything you can see i've missed

i get these errors:
