How Did You Hear About Us Module Broke New Customer Email

OK, I installed the "How did you hear about us" module on my cart. I just realized I installed the version 1.3.8 module and only have a 1.3.6 cart. This broke my welcome email. Any ideas on how to fix it?

After I realized what I'd done, I re-imorted any files that the module over-wrote and turned off the option that requires the referral (since the referral option is no longer on the page) but it's still broken. Please help. I'd like to keep the module, but it's not worth having a broken email. In the meantime, I have taken all the coding out of the welcome email (I have it backed up). But I'd like it to work properly. Thanks in advance!
