Lets say the customer chose a t-shirt (color and size) and the next step is to chose the design to be print on. On the cart/check out, i have to see the connection between them so the customer will be able to order more then one t-shirt each order.
There is any way to do that easy?

Not accurately. Why not make the designs attributes of the T-shirts, since in real life that's what they are?
