ExpressCheckout error [handlingamt]

There is an open ticket with paypal on this issue. Using 1.3.7 with express.

From reference info I have read [AMT] = "total cost of the order. If shipping and tax are known, include in this value."
We get the total, but the AMT appears to be assigned to the wrong line as [HANDLINGAMT].
Can that be changed in php?
PS. we sell ice cream online, so...
