Desperately need help:(

Hi,
one product has 2 different kind of packaging
A) sale by dozen (12pc) chose any 12 colrs
B) kit sale (36pc with display)

I created a dropdown menu for packaging a and b
I have no problem setting this up till here.

when they choose A there should be two seperate text box so they can enter the product number on one and product quantity on the other one so in total they could choose 12 assorted necklaces...
ex: if they choose A from the drop down menu (which is sale by dozen)
Quantity Color
3 blue
5 red
4 green
