Attributes Stopped Working - Please Help!

Hey everyone.

My online store sells a few products which require the customer to select which version of a particular product he/she wants. You can get an example here:

We've been using this setup (without incident) for over a year. The problem is when someone selects this product (and then defined which size and finish they want) - the attributes are not showing up in the order.

About a month ago we installed a patch to inventory our attributes (which did not work the way we thought it would). My designer removed it, but we can't seem to get things back to the way they were.

Does anyone have any thoughts?

It appears to me that the opening and closing form tags are not surrounding the Attributes so when added to the cart, they are not part of the form that gets submitted ...
