No form link from confirmation email for Fax-Your-Payment mod

I am using the fax module email updated from Merlin, I noticed while testing the mod that I get the following confirmation email but with a link:

Fax/phone your Credit Card Info

Please phone your credit card details To: _-___-____
or
Please Fax your credit card details To: _-___-____

Your order will not ship until we receive and have correctly processed your
credit card details.

You can print a fax submission form by clicking on the link above.

I followed all instructions, but for some reason I still cannot get the form link to show up, I was wondering if any else is using this mod and how did they fix it.

Never mind, I have too many payment options as is.
