Discount Coupon Box Showing Twice

Sorry to bother you guys, but I was wondering if anyone can advise me on my problem.
Some how I seem to have gained a second discount coupon box which appears on Step 2 of checkout. The top box doesn't work if you try and put in a coupon number...the lower box does work. I would really appreciate it is someone could tell me where abouts I need to look to get rid of this imposter first box so that my customers don't get confused and think the coupons aren't working.

Just a bit of extra info, I just did a view source and it clearly shows two identical bits of code for the discount code...if only I knew where to find where to delete one.
I tried turning the discount coupon module off via admin/modules/order total...and that got rid of both, but when turned back on both re-appeared.
I don't know if that helps anyone to know what I need to sort out the problem.
