There's a remote possibility that the module may have some corrupted PHP code. It was "clean" php originally (allowing you to upload data), but somehow got corrupted recently (and now is not writing properly to the dbase.
This MIGHT be a solution - but wait to see if others post on your problem before rushing off...
1. Make a TXT file containing all of your settings for this shipping option, so your TXT file will contain lines such as:

2. REMOVE the module, FTP a fresh copy of the php files to OVERWRITE the existing ones, and then REINSTALL the module - and doing this will erase your current settings.
3. Put back the settings as per your TXT file "backup".
