Custom Forms

i am trying to add a custom "request" form to my cart.

i do not need zencart to process the form (i want to use something like formtools for that) just need to display it on an ezpage or popup.

can some one point me in the rt direction to accomplish this?

You can simply add your form script into page_2, page_3 or page_4 in html. When you view that page you will see the form.
