Tax not being charged!?

We just had an order come through from our home state (NY) and for some reason, tax was never charged. Yet every time I do test sales under my name with different zip codes in NY, the tax works just fine. Any ideas?

And you tried to recreate the exact order on your tests with the exact customer information?

If you make a customer identical to your customer who made the order with no tax and look at the database tables in phpMyAdmin for the:
customers
address_book

do you see anything different?
