Charity Donations with 'opt-out'

My client is asking for a facility to add a few Euro charge to every sale as a donation to injured servicemen, with the facility to 'opt-out' if not in agreement.

Is this possible and how would I do it?

I know you asked "how do I" and not "should I", but something like that would cause me to leave your client's site, regardless of the worthiness of the cause. When you want more money from me, it should always be opt-in.

I agree with what you say, but I guess my client feels it will not put off his customers as the majority are people within the armed forces anyway!
