If you were to do that, you would be opening yourself up to massive spam attacks because the tell-a-friend would become an open email relay.

Isn't there the same possibility with any form on a website such as contact us, etc.?
Is there a way to prevent that - such as entering a code displayed on the page, etc.?

The Contact Us is only sent to the Store administrator -not to mass email lists.
CAPTCHA can be added, but is not very effective.

How would I change the code to remove the requirement that user be logged in and add CAPTCHA so any visitor can use tell-a-friend?

Step1: Remove functionality requiring customer to be logged in to use?

Step 2: add CAPTCHA.

Admin -> Configuration -> Email Options -> Allow Guest To Tell a Friend, but you're making a big mistake.
The captcha mod(s) in Free Software Add Ons come with instructions.
