Is there a way to only have a "pickup" or "ship" option in the checkout? There would be a flat rate associated with each product.
We have large items that don't have the same options as most products.

The supplied shipping modules deal with all products in your cart equally -- no exception is made for certain products vs others.
If you want per-product shipping rules, you'll need to either write custom code or see if there's an addon that works for your unique requirements.

All of the products have their own unique shipping cost associated with it. There aren't any exceptions.
What would I need to modify for this?

For a per Product shipping rate, you might look at the Per Unit perweightunit shipping module that utilizes the products_weight field as the shipping cost per quantity of the Product ...
So if the weight is 3 and you have the Rate of 1.25 and you order 4 you get:
(3 x 4) * 1.25 = 15.00
If the weight is 1.25 and you have the Rate of 1.00 and you order 2 you get:
(1.25 x 2) * 1.00 = 2.50
NOTE: be sure to set the Maximum Weight shipped to something high like 5000 and the Tare to 0:0 ...
NOTE: if you do not charge based on Quantity of the Product, then you would need to do quite a bit of customization ...

This sounds like it could work.
So, if
Product A has a shipping cost of $125 (based on weight) and
Product B has a shipping cost of $250 (based on weight)
When the customer orders:
2 of Product A ($125 * 2 = $250) and
1 of Product B ($250)
The Shipping Cost will be $500
($250 + $250 = $500).
Is that how it would work?
That is correct ...
Set the weight on Product A to 250 and the weight on Product B to 500 and using the Per Unit perweightunit shipping module that should work for you with the changes listed on the Shipping/Packaging settings ...

Does the RATE need to be the same throughout?
Can the rate be different by product or by category?
When using Per Unit perweightunit shipping module, how would the shipping display on the product detail page? We want to have a Shipping & Handling heading with a dollar amount shown.
What we want to be able to do is have the dollar amount in the shipping class id field and use that as a shipping & handling amount for that product.
Then at checkout, use that as the basis for the shipping charge.
For example Product A cost $525 and has a shipping cost of $54 (in the shipping class id field)
At checkout it would be $525 + $54 (shipping & handling) = $579
