Default Adding/Making a custom auto form

I'm looking for some type of auto form for one of my ez pages. I made an option for my customers to put in a request for a specific item if the site doesn't have it. For now they have to email me, but it would be nice and easier for the customer if they could just fill a form out and submit it through the site.

The form could include customer contact info(name, email, phone), a manufacturer line, product name line, part number line, and comments box. Then just submit.

Any help finding a custom form like this would be greatly appreciated.

I tried to transfer the contact us section code and it didnt work. I'm still a beginner at this so maybe I did something wrong.

Not sure if that would work anyway?

Just searching through the forums for 'custom form' makes me think it might be easier to do that with one of the extra define pages (page 2, page 3, etc.) than an EZ page.

For a commercial solution, try CoffeeCup Web Form Builder. (There is a discount coupon code available.) You design your form yourself and then upload it to your website root directory. Custom code is generated for loading onto an EZ Page.

This will let you put a form on an EZ Page. Pressing submit can email the form to an address of your choice.

I have no connection with the company, I just bought the product.

There is a 30 day free trial.

Another form builder from a outfit named phpjabbers. You can create as many different forms as you like.

I've haven't tried the form builder, but I have purchased several other scripts that work just fine with ZenCart and WordPress.

    Great support I might add.

Got the form builder. Very easy for a noob and looks good for the first one. I can change things later if I want but this will do for now. Thanks!
