How did you hear about us. Add on

I am not sure if this is the rite place to ask this question but
I am trying to install the "How did you hear about us" add on
The problem is when a new customer would try to log in there is no fields to input the information. Then when hitting the submit button they come up with a warning telling them that they need to fill in the fields about How did you hear about us. What did I do wrong by not having those fields showing up in the log into page.

How could I do a simple disable of this add on until the problem could be located. I would like my new customers to be able to log on.

Forget the help. I figured it out. I forgot to load the last tpl file.
