Template Selection Page not Showing up

Hi-
I created a new template for my website and when i went into Admin to select that template (under Tools --> Template Selection), the page comes up blank and will not show anything on the screen. It is the only page in my admin that wont open. What can I do to preview that page? I am using the latest version 1.3.9. A screen shot is attached.

I have all the folders in my new template correct too so im not sure what the problem is... See attachment and also here are the contents of my templateinfo.php

Look in your /cache/ folder for the files named myDebug-adm-xxxxxxxx.log.
What's in those files?

Ok.. after i posted the contents of my templateinfo I saw that I have an apostrophe in the author name (Jackie's Jewels)... So I deleted that and now the template selection page opens ... However now im getting this error after i select my new template:

Forbidden

You don't have permission to access /newshop/includes/templates/jewels/images/ on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

Did that message appear merely as a result of choosing the template from the pulldown?
Or did it happen when you clicked on the Screenshot Preview link?
Also, what happens if you actually enter a filename for the $template_screenshot ?
