It looks like I have been getting this "PaymentActionNotInitiated" message since the day I installed 139c, May 23, but failed to notice until today.

As you probably already know, PaymentActionNotInitiated is not a problem. Its a normal part of the process? So this is not a bug. I will open another issue with the appropriate complaint that I thought was related with the PaymentActionNotInitiated.
