Site Feedback?

Does anyone know of an addon that will give me the ability to allow my customers to provide feedback about the site via a survey form?

I prefer it NOT to be a sidebox but simply a link that they can click in the footer.

I'm not aware of an add-on to do this but I have an idea of how you might be able to do it (un-tested).

A while back I read a blog post by Erica Douglass that shows how to create surveys using Google Docs. It was here..

If I remember correctly Erica used Google Docs to create the survey and then embedded it into a Wordpress page using an IFRAME. You should be able to do the same except use an ezpage with an IFRAME instead. You can then add the link to the ezpage to your footer.

I've not actually tested this but I think it should work.

Thanks for the reply. Sounds do-able. I was kind of hoping for a pop-up javascript type scenario, but that sounds like it might be an easier implementation until I can get a pop-up like scenario done.
