PHP Notice: Constant IS_ADMIN_FLAG already defined in admin

I'm getting:
PHP Notice: Constant IS_ADMIN_FLAG already defined in /my/path/admin/includes/application_top.php on line 27

Any clues to the solution?

The only time I have come across that error is when I placed a file in the wrong location. Check your other folders in your admin and make sure it wasnt accidentally placed in one of those locations and make sure you didnt put your /includes file in the /admin/includes or vise versa.
