I am testing with a fresh version of 1.3.9h.

The database I upgraded (the 1.2.1 database) went smoothly using the zc_intall folder.

I may have made some small changes to the original 1.2.1 database (via some installed modules). Should I put the database back to 1.2.1 form before upgrading? Do you think that would help? Or should that not matter?

Can not comment as any custom work file or DB is not addressed in an upgrade - - ZenCart upgrade script is not aware of your custom changes

Fair enough. Do you have any suggestions or pointers on where I should start to troubleshoot?

If records added to a table or a new table added the fresh set of files will not be addressing any custom records or tables so there should not be a problem

As you are seeing an issue,, You need to review what you have changed

Compare all your files vs originals, and compare your database schema against the master schema. Fix whatever's mismatched after you figure out why.

Ok I think I resolved it.

I upgraded a clean copy of a 1.2.1 database to the current version, and everything worked perfectly so I knew it was a database issue.

Then I analyzed the differences between a fresh copy of 1.2.1 and my current 1.2.1 data schema. It turns out I added customers_fax in the orders.php outlined in this guide:

So I just followed this guide again with my fresh 1.3.9h files and upgraded my database again and it worked!
