add customers,make orders

hi,
ZC version;
Zen Cart 1.3.8a
Database Patch Level: 1.3.8
v1.3.8 [2008-12-14 11:45:41] (Fresh Installation)
v1.3.8 [2008-12-14 11:45:41] (Fresh Installation)

I just installed the add on "admin_simple_create_order_1-00"
seems to be there but when i make a order(click on "Create Order "Button)
i get this error;

nothing shows up in the error log and no error log is made.
in cart/cache and yes i do have the error logger installed

anyway , just curious but why is this important??
its all fairly up to date....

HTTP Server: Apache

The 1054 error can be a php or mysql ver issue
Can also be a file that causes the in compatibility
