Need help with setting up a single page.

Hi everyone,

I have a e-commerce florist website (zend cart), i notice that after i check out, i can't able to leave a message in the message card for my flower. Hope you all get what i mean.

I would like to setup a single page that i able to leave a message before check out. So after check out the message will send to the administrator and the administrator will know what to write on the card that is with the flower to the people who need to be deliver to.

I really hope you guys able to help out with me. I know abit of php, so i might need help by setting up step by step tutorial.
