Here's what I came up with. I call myself checking to make sure that other options that affect calculations were not set but I could be mistaken.

Handling fee: US and International

US: Handling fee set @ $5
1st line displayed adds $5
2nd line displayed adds $10
3rd line displayed adds $15
4th line displayed adds $20
etc.
Same for shipping quote or during checkout
Same whether logged in or not
Same whether retail or online

International:
First class USPS letter or package appears to add 18 times the International fee
Flat rate appears to add 10 times the International fee

Try changing the "+=" to just "=" on the two lines in this section around line 361:

        if ($order->delivery['country']['id'] == SHIPPING_ORIGIN_COUNTRY || $this->usps_countries == 'US') {
          // domestic/national
          $usps_handling_fee += MODULE_SHIPPING_USPS_HANDLING;
        } else {
          // international

formatting of individual handling fees:
$ sign being on a different line instead of beside the fee input field is hard on my eyes but doesn't affect functionality.

Try changing line 1300:

Looks great in FF & IE9

Chrome & Safari (I don't use either) still looks buggered up w/that issue and overlapping text/input boxes (at least the setup on my monitor)

Try changing all the:
width:50px

to read:
width:60px

in that function ... does that clear things up for you?

changed to read:
