Well, I've had to uninstall the module from admin for now as it completely removes the flat rate option for all products/categories when it's enabled. Did you not see anything obvious in the code then?
I'm assuming the UK zones are set up correctly as the table.php module works just fine with the Shipping Zone set to UK.
It just seems that this new module is interfering with the original one. They don't work together. As soon as I add the new code to table409.php then both of them stop appearing.
What version of Zen Cart are you running?
I was able to get both Table and Table409 modules to work in my v1.5.1 ...

I would love to upgrade, but with a site that's so heavily customised I can see nothing but trouble with that. I'd need a complete guru like you to do it **hint-hint-wink-wink** there would be a barrel of coffee in that one :)
I dug up an ancient v1.3.8 and other than error logs due to not be updated for php 5.whatever ... this is working fine there too ...

The only shipping mods I use are these:
So, we use Special Delivery, Special Delivery 9am, Table and Table International.
I've never understood why Table and Table International have green circles as opposed to the blue ones. They're all four installed and operational.
Could you please assign a valid, unique Sort Order to all Shipping modules that you are using and not use 0 as you have Sort Orders set ...
Then try turning on table409 and see if they are working ...
