Option to purchase cooler bag when going through shipping options

I installed Extra Order Fee as you suggested and yes I think that is the basis of what I want. What I would like to do is have the basis of the extra order based on an answer to a question prior to completing checkout ie "Do you want to purchase a cooler bag for the food - if the answer is NO you commit to putting out a cooler bag or box?". Any ideas on how I achieve that?

I think I get what you mean and this would work by offering a bag as a cross sell item each time. The problem I have is that this would become tedious pretty quickly. This might work if I could integrate the better together module with the shipping module but this is probably beyond my skills. Thanks for the post.
