Create Custom Page with Email Form

I'd like to create a second contact form (separate from the default) which I can ask for a few simple questions (name, address, message, etc.). I'll need to create another page and another form. How do I do this easily? I can write my own PHP, but I don't want to. :)

We've used MachForm forms in EZ pages. You could also place your own HTML form code in an EZ page.
