All this text appears on every page after adding Flexible Returns Plugin

Not sure where to post this problem this issue started when I installed the Flexible returns module.

Use the form below to submit your request.

This content is located in the file at: /languages/english/html_includes/YOUR_TEMPLATE/define_returns.php

You can quickly edit this content via Admin-> Tools-> Define Pages Editor, and select define_returns from the pulldown. You can also edit it directly if you prefer.

NOTE: Always backup the files in /languages/english/html_includes/YOUR_TEMPLATE

shows above the logo on all pages including the index. I have tried to delete the page in the template and in the default template but it still shows up. You can see the page here http://desertpokersupplies.com/
