Contact Us Page not working?

Hi all, any help will be appreciated.
I am in the process of backing up and will slowly upgrade to latest installation of Zen Cart. But in the mean time, my contact us page decided to stop working. I can't figure out where to fix it, and was hoping someone could help me.

It shows up as blank. Any help is appreciated, and apologies for my lack of knowledge in this. Thank you!

Any error log files in your /logs folder?

I don't have the logs available to me since my zen cart is not up to date. Like I said, I will upgrade soon. Any idea where this file is located so I can fix it?

In that case, look in the /cache folder.
