The Contact Us Form Doesn't appear

I have a web site in two languages (English and Hebrew) and I am in the process of updating my site. On the English version all is well for now, however in the Hebrew version in the contact us page the form doesn't show. I have tried going through the files but i can't find where the file which calls for the tpl_contact_us.php is, really I just don't know why the form won't show
I would appreciate any help I can get.

I have fixed the problem and am updating this for future reference to other zenners with a similar problem.. Turns out my define_contact_us.php was under html_includes/classic but didn't appear directly under html_includes or html_includes/YourTemplateName and therefore wasn't being called upon, apparently this is the file which calls on the tpl_contact_us.php file.
