How to add an instruction note (edit text) for a product?

My website (http:www.myphototoy.com) allows customers upload their photo so I can make personalized toys for them. Since it involves personalization, I'd like to add a text box so the customer can enter instructions when they upload the photo (so they can instruct me to, say, use the female in the photo that has a female and a male). How do I add a text box to a product?

You are adding a text option name similar to the upload file option name used for the product. It's another attribute to be associated.
