I also tried to use the "classic" template, but I encounter same issue.
I contacted the plugin author, and he says he is not following it anymore thus not being able to give support, he states it is working on their site they develop for.

Ok problem solved. It was some hosting issue. When/if they tell me what it was I'll update this for others eventually experiencing similar issue.
As promised: hosting says it was related to permission settings on their end.
