Do you now see on the USPS module the Debug Log setting?
If so turn that to Log and try a quote ... then Zip the Log file(s) and attach them to your post ...

that is on your server, what does this line at the top of the file read?

The symptoms you've been reporting all day are typical of using a module which is not properly configured, or has been replaced with a different version, but hasn't been reconfigured and therefore is unable to function as intended.
When Ajeh said to "uninstall then an install and set your settings up", she meant:
1. In your Admin, go to your USPS shipping module, and write down all your current settings..
2. Then click "Remove". (That's the "uninstall" part.) This will remove all settings, just as though the module was never installed.
3. Then click "Install". This will re-install all the newer-version configuration setting options.
4. Now reconfigure all your settings, and any new settings that came with the new module.

In the Modules ... Shipping ... click on the USPS module and click on UNINSTALL ...

Thank you all for all of your help getting this situated. The following steps have been taken:
-Uninstall shipping module
-Download latest version of the plugin (from the link Ajeh gave)
-Upload both files from the extracted folder
-Re-install module and reconfigure.
This is producing the same errors and still is broken.
On the server, are you loading the code file into:
/includes/modules/shipping/usps.php
and the language file to:
/includes/languages/english/modules/shipping/usps.php
