State field showing twice

If you enable pull down state State field is shown twice. Not sure if bug or my settings wrong:

The checkout_flow page is not part of the current built-in Zen Cart distribution. I'd suggest that you post your question in the support-thread for that checkout-changing plugin.

I think it is part of zc1.60

You posted the same thing on github; Using the dev version from GitHub will give you the unreleased v160 code, which has the problem, and the code isn’t ready for release yet.
Using the official released version (v155 currently) will not show that problem.

You've posted in multiple places that you want to develop a payment module. That's wonderful.
But you should be developing it for the current version of Zen Cart: 1.5.x

So the point of my reply above was that you've downloaded the wrong version of Zen Cart to build your plugin upon.
Grab v1.5.5 (max PHP 7.1) or v1.5.6 (supports PHP 7.2) to continue your plans.
