with regards to phone number:
admin -> configuration -> customer details -> telephone number -> set to true
now the phone number will be required and associated with the customer (as opposed to the order.... small difference...)
with regards to how did you find our site, i would look into a customer survey plugin that happens AFTER the sale. something like this:

when taking orders, it is my opinion that you should make it as EASY as possible for a customer to enter an order. burdening them during the sale is a sure way to kill sales, IMHO.
in addition, if you install google analytics or the like, i think it will provide more accurate information than querying the customer. querying the customer is good for feedback on use, as opposed to the how did you find us.... which when properly configured, analytics can tell you.

The How Did You Hear About Us module might come in handy and should work with any 1.5.xx version of Zen Cart.

In a default configuration of ZC, a non-zero value for the configuration->minimum values->telephone number will require the customer to enter a value that has the provided minimum quantity of characters when they are manually creating a login profile. Other methods of account generation may be different.

Ok, we have an issue. We added a question into the buying process & their answers are showing up on the zen site, but not in the e-mail.
How do we get the info into the e-mail?

You can add more data into the email composition in the send_order_email() function of the order.php class. You can add more language defines to the email_extras.php language file. If you are sending HTML emails then you will also need to add the corresponding placeholders to the order confirmation template in the /email/ folder.
