Are you sure you're pointing to the right database? The fields you posted look correct for Zen Cart 1.5.2 or later, but that debug-log indicates that the 'status' field is missing.

Only the status field allows null or none even though it is normally a 1.

Have you checked to make sure the other five fields are populated for all the countries?

You need to keep iheart banner script from loading on a secure page or turn it off all together... causing many script errors which is blocking the call to refresh the zones on first page load.

blocking the script lets me see and select country and state... but the US is missing!! could be the errors that are still getting created... kill iHeart and see what else may be an issue..

In this check of the correct database, are the tables having the correct DB_PREFIX being looked at? What are the contents of your includes/configure.php except for the DB_USERNAME and DB_PASSWORD?
