Shipping rates Based on Table Quantity and State

Greetings,

I'm looking for a shipping scenario, hopefully using existing plugins, to charge shipping based on a table qty rate and location by US states/territories etc. Also would be helpful if there was an All Other rate as well.

EX:

CA, OR, (western states)
1-6 item rate - $20
7-20 item rate - $15
21+ items, free shipping

CO, WY, (mid-west states)
1-6 item rate - $25
7-20 item rate - $20
21+ items, free shipping

NY, VA, (eastern states)
1-6 item rate - $30
7-20 item rate - $25
21+ items, free shipping

All Other
1-20 item rate - $50
21-50 item rate - $45
51+ items, free shipping
