How do I create an easier URL for a page on our site?

This may be more of a general website building question that something specific for Zencart.

We've recently added a survey to our website to get feedback from customers. The author of the survey says we can use:
www.lm-herbs.com/index.php?main_page=survey
to get to the survey.

We would like to be able to send this link to customers in the form of a flyer we put in the package, but that would be too much to put on a flyer and too much to expect the customer to type or remember. We would like to have it be something simple like:
lm-herbs.com/survey
and have that redirect customers to the survey.

How would I best go about accomplishing that?

You could just do an .htaccess redirect from a shorter name (lm-herbs.com/survey) to the canonical name of the Zen Cart page.

I've had people say to use bit.ly or some other url "compressor" to redirect where the remainder of the link is easy to remember. This and the previous suggestion are good for "one-offs", but if there are to be a lot of such links, may want to go with something that would end up being more manageable.

I am afraid I am not that good with computers nor have I actually learned any of this. I found this website:

and it generated the following code:

Redirect 301 www.lm-herbs.com/index.php?main_page=survey lm-herbs.com/survey

I am just going to assume I did this wrong.

The process of taking the code and making it an htaccess redirect is copying the code into Notepad, saving it as a text file, upload it to the website and rename it into an .htaccess file. (I am going to assume I don't have that quite right as well). Where in the directory would I upload that file to?
