Key issues or things to address/handle:
Option for additional images is turned on.
Template supports additional images (two provided templates do support it.)
Then it pretty much comes down to file names.
Recommendation: no spaces or special characters in the file name. Use lower case for file extensions or at least be consistent. Also, it is highly suggested that all of the images of a product be of the same type, i.e. all jpg, all tiff, etc...
All additional images for a product should incorporate the main product's file name as the start of the filename, then use an underscore and some piece of information to help you to identify the additional image.

From those basics: if going to have a lot of photos, then recommend placing them in sub-folders so that no one final folder holds a huge number of images. All images for a product should exist in the same folder/sub-folder.
If you want to show a specifically different image for the medium and large images, then there is a sub-folder that already exists to house such images, though within each of those folders, the same sub-folders are needed as the main images folder.

There is a program that helps sort this out to an extent, Image Handler. The appropriate version depends on your Zen Cart version primarily. IH4 is for Zen Cart 1.5.0 through 1.5.4. IH5 for 1.5.5 and above (for now). This program will basically rename your additional files to show as additional images. IH also optimizes the images so that they are offered to the visitor no larger than the resolution at which they are to be displayed. This saves in band width and speed of loading.

There's also at least one additional plugin that will display additional images with its main focus just on that: additional images.
