No changes were made, has worked for years. Upon discovering problem, I updated the the UPS module https://www.zen-cart.com/downloads.php?do=file&id=1293 from 12-30-2015 to 1-14-19 with no improvement. Host claims no changes also.

I can't "speak to" the behavior of that shipping method, but the UPS XML (https://www.zen-cart.com/downloads.php?do=file&id=126) shipping method appears to be returning quotes. You'll need to copy over any UPS credentials from the 'UPS' module.

My site also had the problem too. zencart v1.56.c with the latest ups module (13 Jan 2019).
How could we debug the issue with some basic php knowledge?
