This can happen for a number of reasons:

Also check all of your store settings from
customers to
store location to
shipping/packaging to shipping restriction settings per shipping module. Review your server response location as I have had a problem with server reporting location beyond Zen stock restrictions in the cart.
Log in and review a customer account in
My Account (using login as customer from admin with master password) to be certain the address locations are set correctly and there are no corrupt or mismatched regional definitions. This has happened in 3 instances along with twice needing to upgrade the javascript within a template as it would not report custom module features delivered by the script.
