This store has been 6+ years in the making?
To obtain assistance, please provide (much) more information... Starting with addressing the posting tips is a good start as right now the 500 error could be for any number of reasons. Likely to have some information though in the logs directory (assuming using some form of recent version of Zen Cart) otherwise older versions (not recommended) would log in the cache directory... Help others help you...

I assume you pulled that from my profile or elsewhere... I had a store many years ago. This is a new store.
I installed ZC using the stock 1.5.6c files manually. DB was also created manually.
As you can see by my post, I'm on 1.5.6c

Just wanted to add to clarify, I have not done any modifications yet. All I've done thus far is creating my list of manufacturers and tax setup. My next steps was product categories and the very first time I clicked on the new category button is the first time I got the error. I feel this may be a typical files issue except that I uploaded the zip in the file manager (cpanel) and uncompressed it and went about my install so it would seem very odd to me that either the zip were to be missing files or that the uncompressing of the zip was at fault. Be that as it may, if the consensus is to re-upload I'm fine with that; I just dont' want to loose my tax zones/rates or my extensive list of manufact

Your missing files that are being called and the missing file is not a part of a stock install

The file it is looking for (includes/modules/prod_cat_header_code.php) was removed in 1.5.6+, the last time it was used was in 1.5.5f
So, what changes have you made?

Okay so when I installed a themeforest template to test it and subsequently uninstalled it (thinking I was starting fresh), there are clearly permanent modifications that were left behind. Well, I guess this further supports my decision to stay 100% stock!
I'm a little rusty to say the least so please bare with me... As I mentioned above, I created all my tax zones and rates as well as my manufacturers. How do I start over and still keep (or restore) these to avoid lost time.
