First Data HCO payment pages module

I just installed and configured the First Data HCO payment pages module v103 on my Zen Cart v1.5.4 store and configured the page and settings on my First Data merchant page. Step 3 'Confirm Order' button in the shopping cart proceed to checkout area doesn't do anything. Doesn't link to the First Data page to process the cc. I'm sure it's a simple fix, just don't know how.

There's a github version from DrByte at https://github.com/zencart/firstdata_hco but it was last touched three years ago.
