Yes, I saw it in the wild - happened because I thought a client had changes to the table but they didn't.

what version of EO were they running?

i have never seen it happen in the wild, and my clients use it quite frequently.

if i edit an order, change nothing, and click update, my version of EO will delete the orders_products record and create a new one, even though nothing has changed.

I think I see the confusion - I'm not saying "the contents of the OP table are unchanged" - I'm saying it's that the *structure* of the OP table has not changed from what was built by zc_install. There are no extra fields in the OP table.
