How to replace sidebox text headers with images?

I have posted a couple of times, and still have not been able to figure out exactly what I need to do to achieve my goal. I have downloaded categories dressing and have managed to get images for certain category headings. You can see it here:

What I am stuck on now is replacing the h3 text for the rest of the sidebox heading with images. I've been at it all day, I've tried the image heading mod thingamajig and when I install that, it makes my site all wonky.

For whatever bizarre reason - this is showing up in each place where I want my images to appear:

Simply Delightful Designs
[home link]

?? does anyone know what that could possibly mean? I've seen a ton of threads on here about replacing sidebox headings with images, but none with any solid basic instructions on how to do this???
