Would keeping sessions in the db or in files make any difference?

Even if 'Prevent Spider Sessions' has been set o true, it still relies upon the spider in question being listed in the spiders.txt file. If it's not in that file then it can generate a session id - unless Force Cookie Use is also set to true.
