how did you hear about us HELP

i tried to download the how did you hear about us contribution for my zen cart v1.3.0. it didnt work and now when a customer tries to register it says
1146 Table 'shopjack_zc3.TABLE_SUBSCRIBERS' doesn't exist
in:
[select count(*) as total from TABLE_SUBSCRIBERS where email_address = 'jackiesaysboo##########################' ]

i dont even care about getting the how did you hear about us working. i just want my site to be back to normal so customers can create accounts. please assist me in fixing this problem
