comment out this code i.e. put // before it
$shipping = $shipping * $shipping_num_boxes;
so that it is
//$shipping = $shipping * $shipping_num_boxes;
I had to then redo the Shipping Methods generator, but after that it worked!!

Commented out line 121... Viola! Table rates working finally.... FINALLY!!! After hours of testing and sandboxing for no reason.. I finally found this thread and it works!

Some progress
Often, a value that you can't identify is the "default" value you send in Carrier or Merchant Calculated shipping....so if it's being used, that will mean Google applied the default value because it couldn't obtain carrier rates or didn't receive a response from your system (respectively).
I had one set to the default value. meaning gco is having issues checking the shipping price with my cart. Probably why the free shipping is showing up too. Any insight to this would be great.
