Getting the above message on install, any help appreciated

What are the permissions on the application_top.php file mentioned?
Did you specifically set any permissions levels when you uploaded?.

Chmod 644 on includes folder and file

I'm not sure what your particular hosting configuration requires, but folders are typically 755 and files typically 644.
